Tournament Registration Checklist:
Only the Team
Captain (or parent/guardian of Team Captain) fills out all information in the online registration form (ONCE PER TEAM).
Each team member must print out St. Jude Fundraiser Form/Waiver.
Each
team member should follow instructions on the Fundraiser Form to collect money (a minimum of $15.00 per player).
Have waiver signed by parent/guardian for each team member (this must be signed and turned in at the tournament).
Money collected along with signed Waivers/Fundraiser Forms for entire team should be turned in
at once when the Team Captain checks in on the day of the event.
